To submit a Visit Summary, please refer to the video guide and steps below:
- Go to "Upcoming" tab and look for the visit you just completed.
- From the visit card, go to the input screen by clicking "Submit Visit Summary" button or "Submit Visit Summary and Claims"
- On visit summary input screen, there are some main sections that you will need to complete:
- Clock in & clock out time: This is captured based on CP action from the app or indicated by the Live Ops team assist from Incident.
- Alertness and Pain level: mandatory field if CR = single CR (optional if CR = Care Facility (B2B))
- Vital Sign: optional
- Assistance provided: mandatory field the required services will be shown by default and allow CP to tick-off if she/he performed question about additional services CP performed if any
- Case summary (mandatory field): this section is for CP report the visit and CR's condition based on timeline
- Photo attachment (optional field)
- Tips on caring CR (optional field)
- Claims section: This section only available if CP submit visit summary within 24hours after visit end time
- After filled in all required information and review the Visit Summary, CP then click "submit" to go to confidential screen
- CP can submit confidential info (if any)
- After confidential information submitted, the "Submit Visit Summary" button will disappear.
