How do I submit a claim?

If you are on a visit that requires a medical escort or shopping and you have to travel using your own, we can reimburse you for this. Click on ‘Submit Summary & Claims’.

Towards the bottom of the page there is a ‘Claims’ section, click on ‘Yes’ and then click on ‘Add Claims Now’. This will allow you to upload a picture of your odometer so that we can calculate your travel mileage.
